All forms in first will be accepted to a maximum of 48 skaters and 4 goalies per division. All others will be put on a waiting list. Please make sure payment is sent to secure your spot. No spot is confirmed until payment is received.

By sending this form as a parent or guardian, you release the members of TSO 4on4 (including but not limited to, the on-ice coaches, and executive) from any injury or accidents that might occur during this summer season.
