Cancellations for any reason within 10 days prior to first day of event will NOT be refunded
N.B. REQUESTS ARE NOT ALWAYS GUARANTEED DUE TO BALANCING.
All forms in first will be accepted to a maximum of 26 skaters and 4 goalies per division. All others will be put on a waiting list. Please make sure payment is sent to secure your spot. No spot is confirmed until payment is received.

Waiver
By sending this form as a parent or guardian, you release the members of TSO hockey (including but not limited to, the on-ice coaches, and executive) from any injury or accidents that might occur during this summer season.
